Addendum: 

April 30, 2012 (Second Infusion)

The patient tolerated up to 300mg per hour of ketamine with stable vital signs.  The patient had no nausea or vomiting with prescribed Zofran and no adverse hallucinations.

The patient showed improvement in pain thresholds associated with an improvement in function.

Liver function tests are pending from Canada.
